An ITIN is a nine-digit number issued by the Internal Revenue Service to individuals that are not eligible to get a social security number but are required for federal tax purposes to have one. The documents required to apply for an ITIN and currently accepted by our office are as follows:

To verify identity (one of the following original document must be presented in person)

• Passport
• Driver license
• State Identification
• Foreign Voter Card
• Medical record (younger than 6-year-old)
• School record (ages 6-17 years-old)

To verify nationality (one of the following original documents must be presented in person)

• Passport
• Birth Certificate

To verify residence in the US (one of the following for dependents living in the USA must be presented)

• Rental Agreement listing applicant’s name as living in the unit
• Utility Service invoice under the applicant’s name
• Bank Statement under the applicant’s name
• School record for applicants younger than 18-year-old
